Polebait haus Waiver of Liability and Assumption of Risk Agreement

Pole Bait LLC

1. Assumption of Risk

I understand that participation in pole fitness and related activities at PoleBait Haus involves physical exertion, use of equipment, and movement that may result in injury (including but not limited to bruises, strains, falls, or accidents). I voluntarily assume all risks associated with participating in pole, dance, and movement activities.

2. Liability Release

I release and hold harmless PoleBait Haus, its owner(s), instructors, and affiliates from any and all claims, demands, causes of action, or liability for personal injury, property damage, or loss that may arise from my participation in activities within the PoleBait Haus.

3. Use of Equipment & Space

I agree to use the poles, furniture, and studio space responsibly and as instructed. I accept responsibility for any damage to equipment, mirrors, or furniture caused by negligence, misuse, or failure to follow instructions. I understand that I may be held financially responsible for repairs or replacements.

4. Respect of Studio Environment

I agree to:

  • Not move or misuse furniture without permission.

  • Avoid having Stain Prone food and drinks from pole stageand furniture.

  • Refrain from wearing body oils, lotions, or jewelry that may damage equipment.

  • Respect noise levels and community guidelines of the space.

5. Health & Safety

I confirm that I am physically able to participate in pole activities. I will inform the instructor/host of any injuries, conditions, or concerns prior to participation.

6. Media Release (Optional)

I consent to being photographed/filmed during this event for promotional use.

7. Agreement

I have read and fully understand this waiver. By signing below, I acknowledge that I am voluntarily participating at my own risk and agree to all terms outlined above.

BY SIGNING THIS RELEASE, I ACKNOWLEDGE THAT I HAVE CAREFULLY READ AND FULLY UNDERSTAND ITS CONTENT. I ALSO ACKNOWLEDGE THAT THIS RELEASE CANNOT BE MODIFIED ORALLY.
